The following information will be needed to send transcripts:
- Name at time of graduation
- Graduation year or if not graduated - the year you attended.
- Date of Birth
- Your contact information in case I need to ask you a question
- Where you want your transcript to be sent - complete with mailing address, or fax or email address
All transcript for this years seniors were mailed to the address given on your exit survey. This process has been completed. Please allow time for your institute to process.
